Senior In-Home Care Coordinator/Scheduler

Golden Independence Home Care , Castro Valley, CA

Golden Independence Home Care helps seniors stay in their homes. We provide services from a couple of hours a week up to 24 hour care. The Care Coordinator plans, organizes and directs home care services for his/her assigned clients. The Care Coordinator is also responsible for scheduling, on-site training and On-Call duties. This is a full time, 40 hours per week position.
Essential Functions
Responsible for initial and continuous review of all aspects of the assigned clients care
Scheduling caregivers for assigned client determine optimal caregiver-client matches
New client and Caregiver Introductions
Ensure caregivers are fully informed and provided necessary information to execute the client s care plan
Main point of contact for assigned clients
A leader & mentor for caregivers - be attentive to caregiver needs, motivate them to succeed & improve
Conduct caregiver Supervised Visits to ensure compliance with all company policies
Fill open client shifts for caregiver call-outs and/or sudden client schedule changes
On-Call responsibilities
Qualifications:
Experienced LNA/PCA preferred
Supervisory Experience Preferred
Ability to handle people and situations during times of stress
Excellent interpersonal and communication skills including written and verbal
Ability to set priorities and give clear directions
Strong computer skills
Ability to problem solve in a fast paced environment
An ability to be flexible
