The Spiritual Care Coordinator is responsible to develop, implement and supervise the spiritual care performed for each client.
My commitment will include:
Providing meaningful spiritual support for hospice patients and families in an organized and consistent manner through the spiritual care team or community clergy.
Providing spiritual care support through the coordination of community, religion or spiritual leaders, memorial services, family assessment and conferences for end-of-life patients.
Consulting to interdisciplinary team, client and family in issues related to spirituality and a religious belief system.
Education: Baccalaureate degree in counseling, theology, divinity or related field from an accredited university is required. Completion of one unit of clinical pastoral education (CPE) preferred or minister services.
Licensure: Must have valid driver s license.
Experience: One (1) year experience in a health care or hospice setting preferred. Experience in family counseling regarding issues of loss, grief, death and dying.
Skills: Ability to develop and maintain rapport with family and clients. Excellent communication skills, both verbal and written. Ability to work well with an interdisciplinary team. Excellent skills in interpersonal relations and organization. Demonstrates problem solving and decision making abilities. Possesses interpersonal skills that promote entry into homes, interdisciplinary collaboration and effective communication, and motivate the involvement of the faith community. Knowledge of community resources and methods for client/family referrals.
